Social Care Navigators (SCNs) play a vital role in enhancing community health networks & reducing health inequities amongst Medicaid Members, including children, caregivers, siblings, & other eligible family members.SCNs are responsible for screening Medicaid Members for unmet Health Related Social Needs (HRSNs) including Housing/Utility; Food Security; Transportation; Employment; Education, & Interpersonal Safety. SCNs are also responsible for screening Members for eligibility of community-based resources, State, &/or Federal resources.
